Health HUB

 

Through a grant from the Nebraska Department of Health and Human Services (NDHHS), SWNPHD works with community partners to increase preventive cancer screenings in southwest Nebraska through a program called Health Hub. This program shares education on preventive health screenings, offers assistance to get these screenings scheduled, and helps people make progress toward healthy living.

 

The focus of the program currently is breast, cervical, and colon cancer screenings among target populations, as well as high blood pressure and obesity as risks for heart disease.

 

 

 

Health Coaching


Health coaching is a proven way of supporting people in the process of improving overall health and wellbeing. Health coaching is done in 3 scheduled phone sessions that work around your schedule.

 

These sessions will focus on:

    • Healthy eating
    • Physical activity
    • Healthy weight
    • Chronic disease management
    • Blood pressure management
    • Tobacco cessation

 

You will receive materials and resources to help motivate you. Health coaches can also connect you with other programs available in your area based on your needs.

 

 

 

 

Every Woman Matters (EWM)


EWM is a federally funded program serving qualified women ages 35-74. EWM pays for office visits associated with Pap tests, pelvic exams, clinical breast exams and lab fees. Age-appropriate mammography and a limited number of diagnostic tests are covered as well.

 

Eligibility Criteria

    • Must be 35-74 years of age
    • Must be a U.S Citizen or Permanent Resident
    • Must meet income guidelines
    • Must NOT have insurance

Healthy Cooking Class


Once a month, SWNPHD and Nebraska Extension partner to host a healthy cooking class in Ogallala. These are free cooking classes to guide you in creating nutritious and budget friendly meals for you and your family.  Previous classes have included recipes for Beef and Broccoli as well as White Chicken Chili. You will get an educational demonstration on how to make the dish, a recipe to take home, and tasting of what was created in class. Watch our Facebook page for the next class.

 

 

 

 

Wellness Screenings


Blood Pressure Screening: SWNPHD offers free blood pressure screenings in our McCook and Ogallala offices during normal business hours by appointment or walk in.


Finger Prick Screening: Through our Target Your Health program, we can tell you what your cholesterol and glucose levels are and provide educational information based on the results.

 

The screening tests for:

    • Total Cholesterol (HDL and LDL)
    • Triglycerides
    • Glucose
